Up to 3 names are included (yours counts as one). Extra names are $5 each. Everyone listed signs their own Form 1583.

3Prepare your USPS Form 1583

Federal rules require this form before we can accept mail for you. You can't sign it online — it gets signed in front of the person who checks your ID — but you can answer it here, which makes the counter visit a signature rather than a form. Don't send ID numbers or photos — just tell us which documents you'll bring.

Opening it for a business? (optional)

A business receiving mail here needs its own Form 1583, naming where it's registered.

4How would you like to finish the ID check?

This part can't be done by web form. USPS Form 1583 requires your two IDs to be checked by a person — either in front of our staff, or by a notary who verifies your identity remotely. That's a federal rule for mail-receiving agencies, not a shop policy, and there is no online-only route around it.
